Every nursing decision you make should be grounded in the best available evidence. Not habit. Not tradition. Not because that is the way it has always been done on the ward.

Evidence-based practice is what separates instinct from informed clinical judgment. It is the process of asking a focused question, finding the best evidence, appraising it critically and applying it meaningfully to improve patient outcomes. It is also one of the most assessed and most misunderstood skills in nursing education.

This learning guide walks you through every stage of the evidence-based practice process, from identifying your clinical question through to reflecting on what you have learned and how it will shape your future practice.

The Clinical Learning Challenge

You have been asked to demonstrate your understanding of evidence-based practice by identifying a patient scenario relevant to your nursing practice, searching the literature systematically, critically appraising two research articles and applying the evidence to your chosen clinical scenario.

Your work must move through each stage of the evidence-based practice process with clarity, analytical depth and consistent academic referencing using the Harvard referencing system.

Your Learning Challenge

Work through the following questions at each stage of the evidence-based practice process:

Stage 1: Introduction to Evidence-Based Practice

  • What are the core principles of evidence-based practice and why is it fundamental to safe, effective and compassionate nursing care?

  • What patient scenario have you chosen to focus on and why is an evidence-based approach particularly important in relation to this specific clinical situation?

  • What does the literature say about the origins, development and current application of evidence-based practice in nursing?

  • How does evidence-based practice align with the NMC Code 2018 and the professional responsibilities of registered nurses?

Stage 2: Search Method

  • Which academic databases did you search and why are they appropriate sources of nursing research evidence? Consider databases such as CINAHL, MEDLINE, PubMed, Cochrane Library and NICE Evidence.

  • What search terms and Boolean operators did you use and how did you combine them to generate relevant, focused results?

  • What inclusion and exclusion criteria did you apply to filter your search results and how do you justify those choices?

  • Which two articles did you select from your search results and why are they the most relevant and appropriate for your chosen clinical scenario?

  • Why did you choose articles that can be appraised using the same critical appraisal tool and what are the advantages of this approach?

Stage 3: Critical Appraisal of Article One

  • Which critical appraisal tool have you chosen, for example the CASP checklist, and why is it the most appropriate tool for appraising this type of research?

  • What is the research design of your first article, qualitative, quantitative or mixed methods, and how does that design influence the way you appraise it?

  • Applying your chosen critical appraisal tool, how would you evaluate the validity, reliability and relevance of your first article to your chosen clinical scenario?

  • What are the strengths and limitations of this article and how do they affect the weight you can place on its findings in clinical practice?

Stage 4: Critical Appraisal of Article Two

  • Using the same critical appraisal tool you applied to your first article, how does your appraisal of the second article compare in terms of validity, reliability and relevance?

  • What are the strengths and limitations of your second article and how do they complement or contradict the findings of your first article?

  • Taken together, what do both articles tell you about the evidence base for your chosen clinical scenario and how confident are you in the strength of that evidence?

Stage 5: Applying Evidence to Practice

  • Based on the evidence from both articles, what specific change or intervention would you propose for your chosen clinical scenario and how do you justify that proposal?

  • How would you implement this evidence-based change in practice and what steps would you take to overcome potential barriers to implementation?

  • How would you involve patients, carers and the multidisciplinary team in the implementation of evidence-based change in your clinical area?

  • How would you measure and evaluate the success of your evidence-based intervention and what outcome measures would you use?

  • What does the broader literature say about the challenges of implementing evidence-based practice in nursing and how can individual nurses contribute to a culture of evidence-based care?

Stage 6: Conclusion

  • What have you learned about the process of evidence-based practice from this experience and how has it changed the way you think about clinical decision-making?

  • What skills does a nurse need to be an effective evidence-based practitioner and which of those skills do you still need to develop?

  • How will you apply the principles and process of evidence-based practice in your future nursing career and what specific steps will you take to keep your practice grounded in the best available evidence?
